Intake Forms

The first time you schedule, you will receive links to the relevant intake forms via email. You can also access the intake forms and submit responses on the Forms page. We use Google Forms to collect your information, using HIPAA-compliant methods.

Note for Minors

For services received by a minor, the parent/guardian must accompany the minor to their first appointment in order to sign a Parental Release Form in person. The parent/guardian does not need to be present for additional appointments for the released services.
